Re-examined patient. Wounds on tail and back appear well scabbed over. Removed several scabs and noted that skin was present under scabs, likely chronic wounds that have mostly healed. On auscultation a grade 4/6 systolic heart murmur was heard over the left pectoral muscle. Rate and rhythm were normal. Abundant white plaques present in oral cavity. Swabbed for cytology. Noted mixed population of bacteria with minimal WBC present. Overall suspect primary disease (cardiac) lead to failure to thrive and immune suppression. Secondary bacterial infection in mouth likely opportunistic. Rec continuing fluids, meloxicam, and TMS and slow feeding to normal plane of nutrition. Once stable and cleared for AI, echo with cardiology to evaluate source of murmur if still present. Re-evaluate oral lesions every 48-72 hours

Wings - Range of motion: Normal Wings - Patagium: No abnormalities Legs/Feet: No abnormalities Legs/Feet- Talons: No abnormalities Neurologic: No abnormalities Ceolom: Empty Vent: Clear, Blood present when taking temperature Droppings: Odorus, pale green Uropygial gland: DNE Other observations: Temperature - 100.0 degrees F Problem list: Quiet, Dehydration 5%, emaciation, soft tissue wounds, oral plaques Tx: 100ml LRS SQ (50 ml/kg + 5% dehydration) 0.19ml Meloxicam (1.5 mg/kg) 1ml TMS (50 mg/kg) Set up on heating pad on high under astro turf/towel. Mite spray to guaze and wiped onto bird. Attempted to feed organ meat. Was interested in eating however it would shake the food out of its mouth, likely due to the oral plaques. Dx: Clinician Examination AI swab sent to State Lab Labwork once stable Assess oral Plaques Wound treatment Prognosis: Fair
